Initial Evaluation/Re-Evaluations $300

In Home Pediatric PT Sessions $150

Report Fee* $50

*If your child has had a PT evaluation in the past 6 months, we may be able to bypass the initial evaluation and go straight to treatment.  You will be requested to provide a copy of the evaluation and upon review, if the content of the report is adequate to initiate services, no evaluation will be required.  However, there will be a $50 report fee to create an updated Treatment Plan for your child based on the report and clinical findings of the first visit. 

Report fee may also be applicable to Letters of Medical Necessity as well.

All evaluation and therapy fees are due at the time of service. Please feel free to contact me directly for more information regarding individual fees. 

Payment and Insurance Information

Thérèse Willis, Pediatric Physical Therapy is considered an out of network provider.  All services are self-pay as we do not file with any insurance companies.  However, while we don’t accept insurance payments, we are happy to provide you with a superbill to facilitate your reimbursement requests.  Please contact your insurance provider to determine what your out of network benefits are.  

Physical therapy services are typically eligible for reimbursement via FSA (Flexible Spending Account), HSA (Health Saving Account), or HRA (Health Reimbursement Account). We recommend contacting your provider to ensure eligibility if you plan on utilizing any of these accounts for PT reimbursement.

We accept cash, check, and electronic payments, such as Venmo or Zelle.
